Summary

Homozygous 5 alpha-reductase deficiency can occur without genital ambiguity. This finding challenges the typical association between this enzyme deficiency and ambiguous genitalia in affected individuals.

Background:

  • 5 alpha-reductase deficiency is an autosomal recessive disorder affecting steroid metabolism.
  • It is typically associated with impaired virilization and ambiguous genitalia in 46,XY individuals.

Observation:

  • Two brothers with homozygous 5 alpha-reductase deficiency presented with penoscrotal hypospadias and micropenis.
  • Despite typical biochemical changes, including elevated testosterone and low dihydrotestosterone, genital development was not severely ambiguous.

Findings:

  • Reduced 5 alpha-reductase activity was confirmed in genital skin fibroblasts.
  • Urine steroid chromatography revealed elevated 5 beta: 5 alpha ratios, consistent with the enzyme deficiency.

Implications:
