Background:

  • Carotid stenosis, a narrowing of the carotid arteries, presents differently in symptomatic versus asymptomatic patients.
  • Differences in disease progression may stem from varying levels of procoagulant protein factors.
  • Tissue factor is a key protein implicated in blood clot formation.

Purpose of the Study:

  • To investigate if symptomatic and asymptomatic carotid stenosis patients exhibit differences in tissue factor expression within carotid artery plaques.
  • To explore the relationship between tissue factor levels and plaque characteristics in patients with severe carotid stenosis.

Main Methods:

  • Carotid artery plaques were collected from 33 symptomatic and 4 asymptomatic patients with >70% internal carotid artery stenosis.
  • Plaques were immunostained using a monoclonal antibody targeting human tissue factor.
  • Stained areas in the plaque cap and core were quantified using specialized computer analysis.

Main Results:

  • A statistically significant increase in tissue factor staining was observed in plaques from symptomatic patients compared to asymptomatic ones (P = 0.0001).
  • Within the symptomatic group, recently symptomatic patients showed significantly higher tissue factor levels in the plaque core (P = 0.003).
  • The study included 37 patients (27 male, mean age 69.3 years, range 53-83 years).

Conclusions:

  • Unstable carotid artery plaques are associated with significantly elevated tissue factor expression in both the cap and core.
  • Higher tissue factor in the core of plaques from recently symptomatic patients may contribute to plaque destabilization.
  • Further research is warranted to fully elucidate the role of tissue factor in carotid plaque instability and stroke pathogenesis.
