Summary

Thygeson superficial punctate keratitis (TSPK) can lead to significant corneal scarring in chronic cases. Topical corticosteroids may be necessary to prevent vision-altering scarring in patients with prolonged disease.

Background:

  • Thygeson superficial punctate keratitis (TSPK) is typically considered a self-limiting condition.
  • Historically, TSPK was believed to resolve without causing corneal scarring.

Observation:

  • This study presents four cases of TSPK with prolonged disease duration.
  • All observed patients developed visually significant corneal scarring.

Findings:

  • Corneal scarring included thickening, Salzmann nodules, stromal opacification, and Bowman layer disruption.
  • One patient required an anterior lamellar corneal transplant due to severe scarring.
  • Pathological findings revealed subepithelial fibrosis and basement membrane thickening.

Implications:

  • Prolonged inflammation in TSPK can result in irreversible corneal damage.
  • Topical corticosteroids may be indicated in chronic TSPK to mitigate scarring risk.
  • This challenges the traditional view of TSPK as a non-scarring condition.
