Area of Science:

  • Orthopedic Surgery
  • Sports Medicine
  • Arthroscopy

Background:

  • Open stabilization procedures show good results for shoulder multidirectional instability.
  • Limited research exists on arthroscopic plication, particularly for overhead athletes.

Purpose of the Study:

  • To evaluate the clinical outcomes of arthroscopic pancapsular plication for multidirectional instability in overhead athletes.

Main Methods:

  • Twenty-three overhead athletes with symptomatic multidirectional instability underwent arthroscopic pancapsular plication.
  • Follow-up averaged 36.3 months, assessing functional outcomes using ASES, Constant, and Rowe scores, alongside pain and range of motion.

Main Results:

  • All patients reported good to excellent functional scores (ASES: 88.4, Constant: 88.1, Rowe: 86.7).

  • Five athletes returned to their previous competitive level; 18 returned to a limited level.
  • Postoperative range of motion showed no significant change, but limited returnees reported more pain and lower functional scores.
  • Conclusions:

    • Arthroscopic pancapsular plication can achieve good shoulder stability in overhead athletes with multidirectional instability.
    • The procedure's effectiveness in enabling a full return to overhead sports requires further investigation.
