US-sponsored research is increasingly conducted in developing nations.
The approach of US Institutional Review Boards (IRBs) to international research challenges is not well understood.

Purpose of the Study:

To explore the challenges US IRBs face when reviewing research conducted in developing countries.
To understand how US IRBs interpret and apply ethical principles and regulations in diverse global contexts.

In-depth interviews were conducted with 46 chairs, directors, administrators, and members of US IRBs.
IRB leaders were contacted from 60 top US institutions funded by the NIH, with 34 participating.

US IRBs encounter ethical and logistical hurdles due to economic/health disparities and limited local knowledge.
IRBs perceive variability in foreign ethics committees and face challenges with cultural differences regarding autonomy, risks, and benefits.
