Summary

Hepatitis C virus (HCV) core antigen testing offers a sensitive and specific method for diagnosing HCV infection, showing promise as a confirmatory test. While not yet replacing HCV RNA tests, it aids in managing HCV.

Background:

  • Hepatitis C virus (HCV) diagnosis relies on antibody and RNA detection.
  • Current methods have limitations: antibody tests have long windows, and RNA tests are costly and equipment-dependent.
  • HCV core antigen (HCV-cAg) is an emerging direct virological marker.

Purpose of the Study:

  • To evaluate the application of HCV core antigen testing in HCV diagnosis and management.
  • To assess the diagnostic performance of HCV-cAg assays through meta-analysis.

Main Methods:

  • A meta-analysis of 25 articles was conducted.
  • Statistical analyses were performed using MetaDisc 1.4 and Stata 11.0.
  • Pooled sensitivity and specificity of HCV-cAg assays were calculated.

Main Results:

  • The pooled sensitivity of HCV-cAg assay was 0.84 (95% CI, 0.83-0.85).
  • The pooled specificity was 0.98 (95% CI, 0.97-0.98).
  • Subgroup analysis indicated higher sensitivity (0.926) and specificity (0.991) for HCV-cAg as a confirmatory test in specific populations.

Conclusions:

  • HCV core antigen assays show high pooled sensitivity and specificity.
  • HCV-cAg testing is a promising confirmatory method for HCV antibody-positive, therapy-naive individuals.
  • Further research is needed to address heterogeneity and its role in definitive HCV diagnosis.
