Performing myomectomy during Caesarean section for fibroids is safe and effective in selected patients. This procedure, even for large fibroids, does not significantly increase blood loss or complications, with good neonatal outcomes.
Area of Science:
- Obstetrics and Gynecology
- Reproductive Surgery
Background:
- Caesarean myomectomy is traditionally avoided due to hemorrhage and morbidity concerns.
- Recent studies suggest myomectomy during Caesarean section is feasible without increased risks.
Purpose of the Study:
- To evaluate the safety and efficacy of myomectomy performed concurrently with Caesarean section.
- To assess perioperative management and postoperative morbidity in cases of Caesarean myomectomy.
Main Methods:
- A case series of 8 women undergoing myomectomy during Caesarean section for large lower segment fibroids.
- Detailed review of antenatal course, surgical procedures, and postoperative outcomes.
Main Results:
- Myomectomy added minimal time (15 min) and hospital stay (1 day) with no significant morbidity.
- Despite large fibroid sizes (7/8 >5 cm), no hysterectomies were required.
- Good neonatal outcomes were observed in all cases.
Conclusions:
- Caesarean myomectomy is a safe and effective procedure for selected patients in tertiary care settings.
- Experienced surgical teams can successfully manage myomectomy during Caesarean delivery, even for large fibroids.
