Background:

Ovarian hernias are exceptionally uncommon, with ovaries and fallopian tubes found in only 2.9% of operable inguinal hernias.
Inguinal hernias in adult females can rarely present with intra-abdominal organs within the hernia sac.

Purpose of the Study:

To report an unusual case of an adult female with an ovary and fallopian tube incarcerated within an inguinal hernia sac.
To describe the clinical presentation, surgical management, and outcome of this rare condition.

Main Methods:

A case study of an adult female presenting with symptoms of an incarcerated left inguinal hernia.
Surgical exploration revealing the left ovary, fallopian tube, and broad ligament within the hernia sac.
Management involved left ovarian cystectomy and mesh repair of the inguinal hernia.

Main Results:

The left ovary contained a hemorrhagic cyst and was found within the left inguinal hernia sac, along with the fallopian tube and broad ligament.
The patient underwent successful surgical intervention including cystectomy and hernia repair.

Conclusions:

This case underscores the rarity of ovarian hernias and the importance of considering ectopic pelvic organs in the differential diagnosis of inguinal hernias in females.
Prompt surgical intervention is crucial for managing incarcerated hernias containing ovarian tissue to prevent complications.
