Human immunodeficiency virus (HIV) patients show white matter (WM) brain alterations. Diffusion tensor imaging (DTI) reveals widespread fractional anisotropy (FA) changes, suggesting common neuropathological processes in HIV-associated neurocognitive disorder and depression.

Main Results:

  • HIV patients with mild lesions (HIV-MRL) showed FA alterations near lesions; those without lesions (HIV-MRN) also had FA alterations.
  • Widespread FA alterations were observed in all HIV patients.
  • Depression was associated with FA increases in reward pathways; neurocognitive disorder correlated with widespread FA reduction.

Conclusions:

  • HIV-MRN patients exhibit FA alterations in regions similar to those with lesions (HIV-MRL), suggesting shared neuropathological mechanisms.
  • Findings support a biological basis for depression in HIV patients, rather than a purely reactive cause.
