Summary

Proton stereotactic body radiation therapy (SBRT) shows promise for medically inoperable stage I non-small-cell lung cancer (NSCLC). This study found proton SBRT to be effective and well-tolerated in an unfavorable patient group.

Background:

  • Proton beam radiation offers potential advantages for non-small-cell lung cancer (NSCLC) treatment.
  • The efficacy of proton stereotactic body radiation therapy (SBRT) for medically inoperable stage I NSCLC is not well-established.

Purpose of the Study:

  • To evaluate the outcomes and toxicity of proton SBRT in patients with medically inoperable stage I NSCLC.

Main Methods:

  • Retrospective analysis of 15 patients with medically inoperable stage I NSCLC treated with proton SBRT.
  • Treatments involved 2-3 proton beams, delivered at 42-50 Gy (RBE) in 3-5 fractions.
  • Toxicity assessed using Common Toxicity Criteria for Adverse Events version 4.

Main Results:

  • Fifteen patients and 20 tumors were treated between July 2008 and September 2010.
  • Treatments were generally well-tolerated, with manageable toxicities including fatigue, dermatitis, rib fractures, and pneumonitis.
  • At a median follow-up of 24.1 months, 2-year overall survival was 64% and local control was 100%.

Conclusions:

  • Proton SBRT is an effective and well-tolerated treatment option for medically inoperable stage I NSCLC patients.
  • Further prospective clinical trials are recommended to confirm the utility of proton SBRT for stage I NSCLC.
