Long-segment thoracoabdominal aortic occlusions in childhood

This study reports on two pediatric cases of long-segment thoracoabdominal aortic occlusion, a rare condition. Surgical arterial reconstructions successfully managed hypertension in both children, with sustained normotension during follow-up.

Background:

- Two pediatric patients, aged 5 and 13, presented with severe, uncontrolled hypertension due to long-segment thoracoabdominal aortic occlusion.
- Both patients underwent complex arterial reconstructions to address the aortic pathology.
Findings:
- The younger patient received an ascending thoracic aorta to infrarenal aortic expanded polytetrafluoroethylene bypass.
- The older patient underwent a distal thoracic aorto-bi-iliac artery expanded polytetrafluoroethylene bypass, including renal artery revascularization.
- No major perioperative complications occurred in either patient.
Implications:
- These cases demonstrate the feasibility and success of extensive arterial reconstructions for long-segment thoracoabdominal aortic occlusion in children.
- Surgical intervention can effectively manage hypertension and improve outcomes in pediatric patients with this rare aortic disorder.
- This report highlights novel surgical approaches for a previously undocumented pediatric condition.
