Background:

Improving discharge care quality is a national priority to reduce readmissions and adverse events.
Understanding how physicians learn to provide high-quality discharge care is limited.

Purpose of the Study:

To explore how physicians learn about high-quality discharge care.
To identify opportunities for improving training in discharge care as a systems-based practice.

Main Methods:

In-depth interviews with 29 internal medicine residents.
Qualitative analysis using the constant comparative method by a multi-disciplinary team.

Main Results:

Residents reported a lack of formal training in discharge processes.
Learning primarily occurred through peer-to-peer instruction and "learning by doing."
Key concepts learned include teamwork, advanced planning, patient safety, continuity of care, and documentation.

Conclusions:

Discharge care is an overlooked area for teaching systems-based practice explicitly.
Unstructured learning leads to variable experiences in discharge care.
Standardized educational interventions can improve systems-based practice development and discharge care quality.
