Optimization of thromboelastography-guided platelet transfusion in cardiovascular surgery

A thromboelastography-guided platelet transfusion protocol significantly reduced platelet use after cardiopulmonary bypass. This approach was especially effective in aortic arch aneurysm repair patients, without increasing bleeding complications.

- Platelet transfusion was significantly lower in the TEG-guided group (22%) compared to the empiric group (48%) (P < 0.01).
- This difference was highly significant in aortic arch aneurysm repair patients (20% vs. 100%, P < 0.01).
- No significant difference in bleeding complications was observed between the groups.

- A TEG-guided PT protocol substantially decreases platelet transfusion requirements after CPB.
- This strategy is particularly beneficial for patients undergoing complex aortic arch aneurysm repair.
- TEG-guided protocols can optimize transfusion practices without compromising patient safety.
