Summary

Etretinate, a vitamin A derivative, can cause bone and joint problems in children with pachyonychia congenita syndrome. Regular monitoring and minimal effective dosage are recommended to manage these rare side effects.

Area of Science:

  • Dermatology and Pediatrics
  • Pharmacology and Toxicology
  • Orthopedics and Radiology

Background:

  • Pachyonychia congenita syndrome (PCS) is a rare genetic disorder.
  • Etretinate, a synthetic retinoid, is used to manage severe skin conditions.
  • Retinoids can infrequently cause bone and joint abnormalities in pediatric patients.

Observation:

  • A 13-year-old girl with PCS experienced clinical, roentgenographic, and biologic signs of bone toxicity.
  • Manifestations included cortical hyperostosis, joint pain, and calcification of tendons.
  • Radiographic findings revealed thinning of long bones, demineralization, and premature epiphyseal closure.

Findings:

  • Etretinate induced significant bone and joint abnormalities in the pediatric patient.

Related Experiment Videos

  • The long half-life of etretinate can delay the onset of toxic effects.
  • Observed skeletal changes suggest a complex interaction between the drug and bone metabolism.
  • Implications:

    • Clinicians should be aware of the potential for etretinate-induced bone toxicity in children.
    • Regular skeletal monitoring is crucial for patients on long-term etretinate therapy.
    • Minimizing the dosage and careful patient surveillance can mitigate risks.
