Urinary Tract Calculi VI: Surgical Management

Procedures for Kidney StonesMedical intervention is necessary when kidney stones or renal calculi are too large to pass spontaneously (typically greater than 5 millimeters) when stones are accompanied by symptomatic infection (such as fever or pyelonephritis), when they impair kidney function, or when they cause persistent symptoms like severe pain, nausea, or urinary retention. Additionally, patients with only one kidney or those who cannot be treated with medical management also require...

Urologic Endoscopic Procedure: Cystoscopic Examination

Meaning of Cystoscopic Examination:Cystoscopy is an essential diagnostic tool in urology that is used to assess the structure and function of the genitourinary system. It provides a direct view of the urethra, bladder, and, in some cases, the ureteral openings. This procedure helps detect structural abnormalities, infections, cancers, and blockages in the urinary tract. There are two types of cystoscopy:Flexible cystoscopy is commonly performed in outpatient settings due to its less invasive...

Summary
This summary is machine-generated.

Ureteric stent placement with an extraction string after ureteroscopy for stone disease does not increase post-procedure related events. Further randomized trials are needed to confirm these findings for ureteral stents.

Area of Science:

  • Urology
  • Endourology
  • Nephrology

Background:

  • Indwelling ureteric stents are commonly used after ureteroscopy for stone disease.
  • 80-90% of patients experience lower urinary tract symptoms, impacting quality of life.
  • Extraction strings can eliminate the need for secondary procedures for stent removal.

Purpose of the Study:

  • To compare post-procedure related events (PREs) in patients with and without ureteric stents with extraction strings.
  • To evaluate the feasibility and safety of using extraction strings for ureteric stents.
  • To address sparse data on the benefits and drawbacks of ureteric stents with extraction strings.

Main Methods:

  • Retrospective review of 181 patients undergoing ureteroscopy with ureteric stent placement.
  • Comparison of PREs (ER visits, unscheduled clinic visits, calls) within 6 weeks post-surgery.
  • Analysis of PREs in patients with (23.8%) versus without extraction strings.

Main Results:

  • Overall, 34.3% of patients experienced PREs.
  • No significant difference in PREs between patients with (37.2%) and without (33.3%) extraction strings (P=0.64).
  • PREs occurred with similar frequency in men and women, irrespective of extraction string presence.

Conclusions:

  • Ureteric stent placement with an extraction string does not appear to increase post-procedure related events.
  • The presence of an extraction string is feasible and does not lead to more complications.
  • Prospective randomized trials are recommended to definitively establish the benefits and disadvantages of ureteric stents with extraction strings.
