Background:

  • Surgery for primary hyperparathyroidism (PHPT) is recommended for symptomatic cases and select asymptomatic individuals based on consensus criteria.
  • Systematic examination of adherence to these guidelines is limited due to insufficient data on nonoperatively managed patients.

Purpose of the Study:

  • To assess parathyroidectomy (PTx) rates in patients with PHPT who meet established consensus criteria for surgical intervention.
  • To identify factors influencing PTx decisions and evaluate adherence to treatment guidelines.

Main Methods:

  • A retrospective analysis of nonuremic PHPT patients (1995-2008) from a large healthcare database (Kaiser Permanente-Southern California).
  • Multivariate logistic regression was employed to determine predictors of PTx.
  • Patient data included symptomatic status (nephrolithiasis), biochemical markers, and bone density.

Main Results:

  • Of 3388 PHPT patients, only 8% were symptomatic. PTx rates were 51% for symptomatic and varied for asymptomatic patients meeting criteria (44% early, 39% late).
  • High calcium levels (>11.5 mg/dL), hypercalciuria, and age <50 predicted PTx, but absolute rates remained around 50%.
  • Bone density did not influence PTx likelihood, while renal impairment predicted against surgery.

Conclusions:

  • Adherence to consensus guidelines for PHPT surgical treatment was not observed in this study population.
  • Parathyroidectomy (PTx) appears to be underutilized in both symptomatic and asymptomatic patients with PHPT, suggesting a gap in guideline implementation.
