Interventions for non-oliguric hyperkalaemia in preterm neonates.

Non-oliguric hyperkalaemia in newborns is common. Insulin and glucose may be better than cation-resin for treatment, but more research is needed for this and other interventions.

Background:
- Non-oliguric hyperkalaemia is a frequent complication in very low birth weight (VLBW) and preterm infants within the first 48 hours of life.
- Defined as plasma potassium > 6.5 mmol/L without acute renal failure, it poses significant risks to newborns.
Purpose of the Study:
- To evaluate the effectiveness and safety of interventions for non-oliguric hyperkalaemia in preterm or VLBW infants.
- Focus on interventions used within the first 72 hours of life, with serum potassium > 6.0 mmol/L and urine output > 0.5 ml/kg/hour.

- Insulin and glucose showed a borderline significant reduction in all-cause mortality compared to cation-exchange resin (RR 0.18).
- Albuterol inhalation effectively reduced serum potassium levels, with no observed differences in mortality or other clinical outcomes compared to saline.
Conclusions:
- Due to limited data from small, uncertain quality studies, definitive clinical practice recommendations cannot be made.
- Insulin and glucose appear preferable to rectal cation-resin for hyperkalaemia in preterm infants.
- Further research, potentially comparing insulin/glucose and albuterol, is warranted; other interventions lack sufficient evidence from RCTs.
