  • The catheter appeared sheared, suggesting a mechanical failure during removal.

Findings:

  • A guidewire was inserted through the CVC, with its J-tip positioned just beyond the catheter tip.
  • The CVC and guidewire were withdrawn together, leading to suspected catheter shearing.

Implications:

  • Guidewire entrapment in a CVC port may cause catheter folding and shearing.
  • This case highlights the importance of careful CVC removal techniques to prevent rare complications.
