Summary

Giardia lamblia infection can mimic celiac disease (CD) symptoms and biopsy results. Successful treatment of giardiasis resolved symptoms and mucosal abnormalities, even without a gluten-free diet.

Background:

  • Celiac disease (CD) diagnosis requires differentiating from other conditions.
  • Giardiasis can present with similar gastrointestinal symptoms and serological markers to CD.

Observation:

  • A young male with diabetes and family history of CD presented with diarrhea and weight loss.
  • Duodenal biopsies showed partial villous atrophy and elevated IgA antitissue transglutaminase antibodies.
  • Stool tests revealed Giardia lamblia infection.

Findings:

  • The patient was initially diagnosed with CD and advised a gluten-free diet.
  • Treatment for giardiasis led to symptom improvement and discontinuation of the gluten-free diet.
  • Follow-up revealed complete resolution of villous atrophy and normalization of antibody levels.

Implications:

  • Giardiasis should be considered in the differential diagnosis of suspected celiac disease.
  • Accurate diagnosis and targeted treatment of parasitic infections can lead to complete recovery.
  • This case highlights the importance of comprehensive diagnostic workups in gastroenterology.
