Background:

Sickle cell disease poses risks of infertility or sterility, particularly after bone marrow transplant.
Ovarian tissue cryopreservation (OTC) offers a method to preserve fertility in young patients.
Ethical considerations are paramount when making medical decisions for pediatric patients.

Observation:

A 2-year-old female, the youngest on record, underwent OTC.
The patient had a rare sickle cell disease requiring a bone marrow transplant.
The decision for OTC was made by the mother for the patient's future childbearing potential.

Findings:

OTC in this pediatric case presents novel ethical and medical considerations.
The procedure was ethically justified using principlism (autonomy, nonmaleficence, beneficence, justice).
This case highlights the possibility of fertility preservation in very young patients facing gonadotoxic treatments.

Implications:

This case expands the understanding of the youngest age for successful OTC.
