Summary

Flexible Spectral Imaging Color Enhancement (FICE) improves capsule endoscopy by enhancing small bowel lesion detection. This virtual chromoendoscopy technique increases diagnostic accuracy for erosions and angiodysplasias without missing conventional findings.

Flexible Colonoscopy in Mice to Evaluate the Severity of Colitis and Colorectal Tumors Using a Validated Endoscopic Scoring System

Background:

Capsule endoscopy (CE) is a key tool for small bowel examination.
Limitations in CE include the inability to modify image acquisition.
Software tools like Flexible Spectral Imaging Color Enhancement (FICE) offer potential improvements.

Purpose of the Study:

To assess the reproducibility of FICE system interpretation in CE.
To evaluate the diagnostic accuracy of FICE in CE.

Main Methods:

Prospective study with 20 patients.
Four physicians interpreted static FICE images for inter-observer agreement (Fleiss Kappa Test).
Two gastroenterologists compared conventional CE videos with CE-FICE videos (setting 2) for lesion detection.

Main Results:

Substantial overall concordance (0.650) among four physicians interpreting FICE images.
CE-FICE identified 95 findings compared to 75 with conventional CE.
CE-FICE detected more angiodysplasias (35 vs. 32) and erosions (41 vs. 24) and did not miss any conventional findings.

Conclusions:

Interpretation of CE-FICE images shows reproducibility among experienced observers.
Virtual chromoendoscopy using FICE in CE enhances diagnostic accuracy.
FICE highlights small bowel erosions and angiodysplasias missed by conventional CE.
