    Background:

    • Postoperative urinary retention (POUR) is a common complication.
    • POUR can cause patient discomfort, bladder atony, and prolonged hospital stays.
    • Understanding POUR predictors is crucial for patient management.

    Purpose of the Study:

    • To analyze variables associated with postoperative urinary retention.
    • To identify patient and procedural factors influencing the need for catheterization.
    • To assess the quality of documentation in the Post-Anesthesia Care Unit (PACU).

    Main Methods:

    • Retrospective chart review of 100 male postoperative patients at a Veterans Administration Medical Center.
    • Exclusion criteria included pre-existing urologic issues, paralysis, short procedures (<3 hours), and indwelling catheters.
    • Data collected on anesthetic type, IV fluid volume, procedure type, and catheterization needs.

    Main Results:

    • 61% of patients required straight catheterization for initial urinary output.
    • Spinal anesthesia (65%) and general anesthesia (57%) both led to significant catheterization rates.

    • PACU documentation completeness was only 10%.

    Conclusions:

    • POUR is a frequent complication, particularly after orthopedic surgery and with high IV fluid administration.
    • Incomplete PACU documentation hinders accurate assessment of POUR predictors.
    • Improved PACU assessment protocols and documentation are essential for managing POUR.
