Background:

Splenic infarct is a known but rare complication of Plasmodium falciparum malaria.
Increased global travel and rising antimalarial drug resistance may lead to a higher incidence of this condition.

Observation:

A case of splenic infarct during P. falciparum malaria is presented.
Imaging findings included a peripheral hypoechoic lesion on ultrasonography and a low attenuation lesion on computed tomography within the splenic parenchyma.

Findings:

Diagnostic imaging suggested splenic infarct.
Spontaneous regression of the lesion confirmed the diagnosis.

Implications:

This case highlights the utility of imaging in diagnosing rare splenic complications of malaria.
