Age- and weight-adjusted warfarin initiation nomogram for ischaemic stroke patients

An age- and weight-adjusted warfarin initiation nomogram (AW-WIN) significantly reduced time to therapeutic anticoagulation in ischemic stroke patients. This warfarin dosing method also lowered the risk of excessive anticoagulation, proving safe and efficient.
