Summary

This study details a successful combined bladder exstrophy and epispadias repair in a 2-year-old boy. The surgical approach achieved good cosmetic results and functional urinary continence, with a minor fistula successfully managed.

  • This case involves a 2-year-old male with untreated bladder exstrophy and epispadias.

Observation:

  • The patient underwent a combined bladder exstrophy and epispadias repair under general anesthesia.
  • Surgical procedures included bilateral anterior innominate and vertical iliac osteotomy due to wide pubic diastasis.
  • A modified Cantwell-Ransley repair was utilized for urethroplasty and penile reconstruction.

Findings:

  • The repair resulted in excellent cosmetic outcomes and good approximation of pubic and rectus muscles.
  • Post-operatively, the patient achieved urethal voiding, with a small penopubic fistula noted and subsequently managed.
  • Urine culture remained sterile, and the patient was discharged on prophylactic antibiotics.
