Background:

Unauthorized medication collections (UMCs) on inpatient (IP) units are a potential source of medication errors.
UMCs are defined as unapproved medications stored for immediate access on IP units.
This study aimed to identify UMCs on IP units and characterize their contents.

Purpose of the Study:

To determine the prevalence of UMCs on inpatient units.
To document the types and characteristics of unauthorized medications found.
To assess the potential risks associated with UMCs in acute care settings.

Main Methods:

A descriptive study was conducted across all 17 inpatient units of a suburban acute care hospital.
Unauthorized medications were identified by searching all IP units.
Medications were classified as high-alert, easily confused, expired, improperly stored, or controlled substances, and their value was calculated.

Main Results:

UMCs were present on every inpatient unit surveyed.
A total of 656 unauthorized medications, comprising 163 unique products, were identified.
High-alert, easily confused, expired medications, and controlled substances were documented; no improperly stored medications were found.

Conclusions:

The universal presence of UMCs necessitates further investigation into their underlying causes.
Strategies must be developed and implemented to eliminate UMCs from inpatient units.
Nurses should proactively identify and remove UMCs and critically evaluate their practice to prevent unsafe medication storage.
