Two different wall-tracking systems (WTS) provide non-interchangeable carotid stiffness measurements. However, calibrated distension waveforms accurately estimate local carotid pulse pressure, suggesting potential for clinical use.

Main Methods:

  • 105 patients underwent right common carotid artery (CCA) assessment using Esaote (QAS, MyLab) and Aloka (E-track; Alpha 10) WTS during the same session.
  • Carotid PP was estimated using both WTS and applanation tonometry in 78 patients.

Main Results:

  • Carotid distension was significantly lower with Esaote (363 ± 162 μm) compared to Aloka (458 ± 176 μm) (P < 0.0001).
  • Beta-stiffness index was significantly higher with Esaote (11.9 ± 5.5) than with Aloka (9.4 ± 3.8) (P < 0.0001).
  • Carotid PP derived from calibrated distension waveforms was comparable to applanation tonometry (42.6 ± 11.4 vs. 43.0 ± 10.7 mmHg, P = 0.51).

Conclusions:

  • Measurements of carotid distension and stiffness from the two evaluated WTS are not interchangeable and cannot be combined in a single database.
  • Calibrated distension waveforms show promise as an acceptable method for estimating local carotid pulse pressure.
