A traumatic arachnoid cyst caused nerve issues in a teen after a motorcycle accident. Surgery to remove the cyst and hardware led to slow but progressive neurological recovery.

Area of Science:

  • Neuroscience
  • Orthopedic Surgery
  • Pediatric Traumatology

Background:

  • A 15-year-old male sustained femur and tibia fractures from a motorcycle accident, treated with immediate osteosynthesis.
  • Post-operatively, he developed radicular paresis affecting the left lower limb, specifically the sciatic nerve, confirmed by electromyography.

Observation:

  • Despite normal fracture healing, neurological recovery was slow and incomplete.
  • A subsequent MRI revealed an arachnoid cyst compressing the L5 and S1 nerve roots.

Findings:

  • Surgical excision of the arachnoid cyst and hardware was performed.
  • Histopathology confirmed a traumatic etiology for the cyst, evidenced by hemosiderin deposits.

Implications:

  • It underscores the diagnostic challenge in differentiating traumatic vs. traumatized cysts and the importance of surgical intervention for symptomatic cases.
