Kidney Transplant II: Surgical Procedure

Preoperative ManagementThe primary goals of preoperative management in kidney transplantation are to optimize the patient’s metabolic state and prepare them for surgery through diet adjustments, necessary dialysis, and tailored medical treatment. This phase also involves comprehensive infection screening and patient education about the surgical procedure and postoperative care to improve outcomes and adherence.Medical ManagementA comprehensive evaluation is required for both the living donor...

Summary

This study developed a retroperitoneoscopic nephrectomy (RPN) training model in piglets, establishing an eight-case learning curve. The porcine model proved feasible for practicing retroperitoneoscopy, enhancing surgical training.

Area of Science:

  • Urology
  • Surgical Training
  • Minimally Invasive Surgery

Background:

  • Retroperitoneal approach is preferred for open urological surgery.
  • Retroperitoneoscopy is less preferred for laparoscopic procedures.
  • A need exists for a standardized retroperitoneoscopy training model.

Purpose of the Study:

  • To develop a training model for retroperitoneoscopic nephrectomy (RPN).
  • To establish an experimental learning curve for RPN.
  • To assess the feasibility of a porcine model for retroperitoneoscopy training.

Main Methods:

  • Fifteen piglets underwent standard retroperitoneoscopic nephrectomy (RPN) using a three-port technique.
  • Intraoperative data including operative time and blood loss were recorded.
  • Animals were divided into two groups (first eight and last seven) for statistical comparison.

Main Results:

  • Fifteen RPNs were successfully performed with operative times ranging from 15-50 minutes (median 30) and blood loss from 5-100 mL (median 20).
  • Five peritoneal openings occurred, with two vascular complications managed laparoscopically.
  • Significant differences were observed between groups regarding peritoneal opening, operative time, and blood loss, indicating a learning curve.

Conclusions:

  • Retroperitoneoscopic nephrectomy in a porcine model effectively simulates the surgical procedure.
  • The experimental learning curve for RPN was determined to be eight cases.
  • RPN in a porcine model is a feasible and valuable method for teaching and practicing retroperitoneoscopy.
