  • Malignant schwannoma is a rare peripheral nerve sheath tumor, comprising 5-10% of soft tissue sarcomas.
  • Cervico-facial localization is exceptionally rare, accounting for only 1% of mandibular tumors.

Observation:

  • A 64-year-old male presented with malignant schwannoma of the inferior dental nerve.
  • Initial resection was followed by recurrence and proximal extension, necessitating transfer to a specialized center.

Findings:

  • The patient underwent wide surgical resection, ipsilateral neck dissection, and postoperative external radiotherapy.
  • No recurrence was observed after a 1-year follow-up period.

Implications:

  • Diagnosis of cervico-facial malignant schwannoma can be challenging, even with advanced imaging; histological confirmation is crucial.
  • While prognosis is often guarded due to recurrence risk, multimodal treatment including surgery and radiotherapy can be effective.
  • This case underscores the importance of aggressive surgical management and adjuvant therapies for rare head and neck malignancies.
