Untreatable headache and unilateral ptosis can be presenting symptoms of complex neurological disorders.

Observation:

A patient presented with persistent headache and unilateral ptosis.
Initial CT scan showed no structural cause; MRI angiography revealed absent left internal carotid artery.
Catheter angiography confirmed the agenesis of the left internal carotid artery.

Findings:

The underdeveloped carotid canal on the initial CT scan's bone window is a key diagnostic marker.
This finding, often overlooked, signifies internal carotid artery agenesis.

Implications:

Recognizing the underdeveloped carotid canal on CT scans can lead to earlier diagnosis of internal carotid artery agenesis.
This aids in managing rare vascular anomalies and associated neurological symptoms.
