Collagenous colitis, a cause of chronic diarrhea, rarely occurs with spondylarthropathy. This study presents two cases, highlighting a potential link between these conditions and suggesting a systemic rheumatic disease connection.

  • Collagenous colitis is an emerging cause of chronic watery diarrhea.
  • Co-occurrence of collagenous colitis and spondylarthropathy is infrequently reported.
  • Understanding this association may offer insights into systemic rheumatic diseases.

Observation:

  • Two cases (33-year-old male, 20-year-old female) presented with pre-existing spondylarthropathy.
  • Collagenous colitis developed 4 and 14 years after the onset of spondylarthropathy in these patients.
  • The female patient also exhibited sicca syndrome and vitiligo.

Findings:

  • Histological examination confirmed collagenous colitis in both patients.
  • The temporal relationship suggests a potential link between spondylarthropathy and collagenous colitis development.
  • Associated autoimmune conditions (sicca syndrome, vitiligo) were noted in one case.

Implications:

  • The co-occurrence suggests collagenous colitis might be part of a broader systemic rheumatic disease spectrum.
  • Therapeutic challenges arise from managing coexisting collagenous colitis and spondylarthropathy.
  • Further research is warranted to elucidate the pathogenesis of this association.
