Area of Science:

  • Neuroscience
  • Neuropathology
  • Neurosurgery

Background:

  • Neuroglial cysts are rare congenital lesions, typically found within the central nervous system, most commonly in the brain.
  • Spinal intramedullary neuroglial cysts are exceptionally uncommon presentations of these congenital malformations.

Observation:

  • A 60-year-old male presented with a year of conus medullaris compression, manifesting as paraparesis and genitourinary dysfunction.
  • Spinal MRI revealed an intramedullary cyst at the T12-L1 level, consistent with a neuroglial cyst.

Findings:

  • Histopathological examination confirmed the diagnosis of a neuroglial cyst following surgical evacuation and marsupialization.
  • Postoperative imaging showed a persistent residual cavity, indicating the difficulty of complete resection.

Implications:

  • This case underscores the diagnostic and therapeutic challenges posed by spinal intramedullary neuroglial cysts.
  • Complete surgical resection of these benign lesions is often difficult due to the lack of a clear cleavage plane.
