Summary
Spinal intramedullary neuroglial cysts are rare congenital lesions. This case highlights the diagnostic challenges and surgical difficulties in managing these benign but complex spinal cord tumors.

Background:

- A 60-year-old male presented with a year of conus medullaris compression, manifesting as paraparesis and genitourinary dysfunction.
- Spinal MRI revealed an intramedullary cyst at the T12-L1 level, consistent with a neuroglial cyst.

- Histopathological examination confirmed the diagnosis of a neuroglial cyst following surgical evacuation and marsupialization.
- Postoperative imaging showed a persistent residual cavity, indicating the difficulty of complete resection.

- Complete surgical resection of these benign lesions is often difficult due to the lack of a clear cleavage plane.
