Irritable Bowel Syndrome II: Clinical Features and Diagnostic Evaluation

Irritable Bowel Syndrome II: Clinical Features and Diagnostic Evaluation
Irritable Bowel Syndrome (IBS) is classified into subtypes based on the predominant bowel habits as determined by the Bristol Stool Form Scale (BSFS). The subtypes are:

Irritable Bowel Syndrome

DefinitionIrritable bowel syndrome (IBS) is a functional gastrointestinal disorder characterized by recurrent combinations of abdominal pain, bloating, diarrhea, or constipation.Pathophysiology of irritable bowel syndromeIts pathophysiology is multifactorial, involving disturbances in motility, sensory processing, microbial balance, barrier integrity, and gut–brain communication. These mechanisms interact to produce symptoms that vary across IBS subtypes.Altered Motility PatternsDisordered...

Irritable Bowel Syndrome I: Introduction

Irritable Bowel Syndrome (IBS) is characterized by functional disturbances in the gastrointestinal system, presenting a cluster of symptoms without evident structural or biochemical abnormalities. It primarily affects the large intestine and may cause abdominal pain, bloating, excessive gas, diarrhea, constipation, or both.
IBS is a chronic condition that can persist over a long period or recur frequently.
The pathogenesis of IBS involves a complex interplay of the following factors:
Altered...

Inflammatory Bowel Disease III: Diagnostic Studies and Management I-Nutritional Therapy

Various diagnostic tests are employed in the diagnostic process for Inflammatory Bowel Disease (IBD), particularly to differentiate between Crohn's disease and ulcerative colitis.
Diagnostic studies
A colonoscopy is the definitive screening test, distinguishing ulcerative colitis from other colon diseases with similar symptoms. During a colonoscopy test, inflamed mucosa with exudate ulcerations can be observed, and biopsies are taken to determine the histologic characteristics of the colonic...
Assessment of the Rectum and Anus01:25

Assessment of the Rectum and Anus

Evaluating the rectum and anus plays a crucial role in conducting a thorough physical examination of the gastrointestinal system. Although it may be uncomfortable and often embarrassing for the patient, it holds immense diagnostic value, particularly in detecting gastrointestinal diseases and abnormalities. This guide will explain how to perform this assessment using inspection and palpation methods.
Rectal Inspection
Begin by inspecting the perianal and anal areas for color, texture, rashes,...

Summary

Irritable bowel syndrome (IBS) patients often present with "red flag" symptoms. However, diagnostic testing for these warning signs in IBS yields few significant findings, suggesting a low diagnostic yield.

Area of Science:

  • Gastroenterology
  • Internal Medicine
  • Clinical Diagnostics

Background:

  • Irritable bowel syndrome (IBS) diagnosis relies on clinical criteria.
  • Specific
  • red flag
  • symptoms warrant further diagnostic testing.

Purpose of the Study:

  • To investigate the diagnostic yield of testing for
  • red flag
  • symptoms in patients meeting IBS criteria.

Main Methods:

  • Prospective review of 200 IBS patients (ROME III criteria).
  • Evaluation for
  • red flags
  • : rectal bleeding, iron-deficiency anemia (IDA), weight loss, family history of colon cancer, fever, late age of onset.
  • Review of diagnostic testing and findings.

Main Results:

  • 139 (70%) IBS patients had at least one
  • red flag
  • symptom.
  • Diagnostic testing was infrequently performed and yielded few significant findings.
  • High prevalence of
  • red flags
  • but low testing frequency and yield observed.

Conclusions:

  • A high prevalence of
  • red flag
  • symptoms exists in IBS patients.
  • Diagnostic testing for these
  • red flags
  • in IBS is infrequent and has a low diagnostic yield.
  • Further studies are needed to confirm the low yield of testing even when indicated by
  • red flags
  • .
