Summary

A 40-year-old man experienced delayed diagnosis of myocardial infarction and dilated cardiomyopathy, initially misdiagnosed as panic attacks. Advanced treatments including a ventricular assist device and dialysis were required, highlighting the need for timely cardiac evaluation.

Background:

A previously healthy 40-year-old male presented with atypical symptoms including dyspnea and chest pain.
Initial presentation was misdiagnosed as panic attacks, leading to a delayed diagnosis.

Observation:

The patient re-presented with worsening symptoms, leading to diagnoses of myocardial infarction and dilated cardiomyopathy.
He required significant cardiac interventions, including a pacemaker, defibrillator, and later a ventricular assist device (VAD).

Findings:

The patient developed end-stage renal disease requiring hemodialysis.
He is currently awaiting a combined heart and kidney transplant.
