Background:

Legionella pneumonia is gaining attention in Japan, yet solitary cases are infrequently reported.
Legionella dumoffii, a less common cause of Legionnaires' disease, presents diagnostic challenges.

Observation:

A 59-year-old male with no prior health conditions presented with fever and cough, rapidly progressing to pneumonia.
Despite treatment with erythromycin, rifampicin, and minocycline, the patient's condition worsened, leading to death.

Findings:

Legionella dumoffii was definitively identified in multiple respiratory and autopsy specimens.
Elevated serum antibody titers confirmed L. dumoffii infection.
This marks the first documented case of L. dumoffii pneumonia in Japan.
