Summary

This case study describes a unique instance of unilateral frosted branch angiitis in a patient with a history of eye trauma. The condition, presenting as pseudoarteritis, showed improvement with steroid therapy.

Background:

  • Frosted branch angiitis (FBA) typically presents in three forms: secondary to infections/autoimmune diseases, related to malignancy, or idiopathic.
  • Atypical presentations necessitate careful differential diagnosis and classification.

Observation:

  • A 58-year-old patient presented with unilateral retinal artery sheathing, occlusion, vitritis, and macular edema following ocular trauma, cataract extraction, and vitrectomy.
  • Contralateral eye and biochemical analyses were normal, apart from slightly elevated antinuclear antibody (ANA) levels.

Findings:

  • The patient's presentation did not align with established FBA classifications.
  • The condition was provisionally termed pseudoarteritis.
  • High-dose steroid therapy resulted in gradual clinical improvement.

Implications:

  • This case expands the spectrum of frosted branch angiitis presentations.
  • It highlights the importance of considering trauma history in atypical vascular inflammatory conditions of the eye.
  • The term pseudoarteritis may be useful for cases mimicking FBA but lacking clear etiological links.
