Summary

The waffle-cone technique offers a new approach for treating wide-necked basilar bifurcation aneurysms. This modified stent method, using the Solitaire AB stent, provides effective neck support for coiling when standard methods fail.

Background:

  • Wide-necked basilar bifurcation aneurysms pose treatment challenges due to anatomical complexity.
  • Conventional stent-assisted coiling may be difficult when bilateral posterior cerebral arteries are hard to catheterize.

Observation:

  • Two cases of wide-necked basilar bifurcation aneurysms (one ruptured, one unruptured) were treated.
  • The waffle-cone technique, involving distal stent protrusion into the aneurysm fundus, was employed with a Solitaire AB stent.

Findings:

  • The waffle-cone technique provided adequate neck support for subsequent coiling in both cases.
  • Successful aneurysm treatment was achieved, with near-complete obliteration in the unruptured case.
