Endoscopic Procedures IV: Sigmoidoscopy and Laproscopy

Sigmoidoscopy and laparoscopy are distinct medical procedures that enable physicians to internally inspect different parts of the GI tract. Although they serve different purposes, each is essential for diagnosing and, in some cases, treating various medical conditions.
Sigmoidoscopy
Sigmoidoscopy is a diagnostic procedure that uses a flexible sigmoidoscope equipped with a light source and camera to examine the rectum and sigmoid colon. The procedure involves inserting the tube through the anus...

Area of Science:

  • Minimally Invasive Surgery
  • Surgical Innovation
  • Gastroenterology

Background:

  • Natural orifice transluminal endoscopic surgery (NOTES) offers a novel approach, avoiding skin incisions.
  • The Department of Surgery in Pilsen has utilized NOTES since 2008, beginning with hybrid techniques in animal models.
  • This study evaluates the pure transgastric NOTES method in a preclinical setting.

Purpose of the Study:

  • To compare the safety and efficacy of the transgastric pure NOTES technique against conventional laparoscopy.
  • To assess infectious complications and clinical outcomes in an animal model.

Main Methods:

  • A prospective study involving 15 domestic pigs undergoing transgastric pure NOTES via a double-channel endoscope.
  • Gastrotomy closure was achieved using OTSC clips.

Clinical Application of Single-Surgeon, Three-Port, Laparoscopic Resection for Colorectal Cancer with Natural Orifice Specimen Extraction

Published on: March 24, 2023

  • A control group of 15 pigs underwent standard laparoscopy, with both groups monitored for two weeks.
  • Main Results:

    • Transgastric pure NOTES demonstrated similar outcomes to laparoscopy.
    • Inflammation at the skin suture site occurred in 6 pigs (40%) undergoing laparoscopy.
    • No focal abdominal infections were observed in either group during follow-up or post-mortem examination.

    Conclusions:

    • The pure NOTES approach is technically challenging but feasible with current equipment.
    • Advancements in technical equipment are crucial for the successful clinical implementation of NOTES.
