Summary

This study found that using reusable ultrasonic shears for open thyroidectomy significantly reduced operating time and permanent hypoparathyroidism rates. Vocal cord palsy rates remained comparable between the ultrasonic shears and conventional techniques.

  • A comparative study involving 508 patients undergoing open thyroidectomy.
  • Patients were divided into two groups: those using reusable ultrasonic shears (SonoSurg group, n=237) and those using conventional techniques (suture ligation and clips, n=271).
  • Reusable shears were autoclaved and replaced after 20 uses; outcomes were compared between groups.

Main Results:

  • The SonoSurg group demonstrated significantly shorter operating times (60 min vs. 105 min) compared to the conventional technique group.
  • Permanent hypoparathyroidism rates were significantly lower in the SonoSurg group (1.9% vs. 9.8%).
  • Temporary hypoparathyroidism rates were also lower in the SonoSurg group (12.4% vs. 25.5%), while permanent vocal cord palsy rates were similar (P=0.262).

Conclusions:

  • Reusable ultrasonic shears are associated with reduced operating time and lower rates of permanent hypoparathyroidism in open thyroidectomy.
  • The use of ultrasonic shears did not significantly increase the risk of vocal cord palsy.
  • This technology offers a potentially safer and more efficient approach to open thyroidectomy.
