    Summary

    The presence of clear cells in hepatocellular carcinoma (HCC) tumors is the sole significant histologic predictor of patient survival. Other features like cell differentiation or architecture did not correlate with outcomes.

    Background:

    • Hepatocellular carcinoma (HCC) is a primary liver cancer with variable prognosis.
    • Histologic features are crucial for understanding tumor behavior and patient outcomes.

    Purpose of the Study:

    • To investigate the correlation between specific histologic features of hepatocellular carcinoma (HCC) and patient survival.
    • To identify prognostic indicators in HCC beyond established factors.

    Main Methods:

    • Retrospective analysis of histologic slides from 80 patients with confirmed HCC.
    • Statistical evaluation of survival rates based on the presence and pattern of clear cells and other histologic characteristics.

    Main Results:

    • Overall survival for HCC patients was 50% at 3.5 weeks, with a significant mortality drop after 8.5 weeks.
    • Patients with diffuse clear-cell pattern showed the best survival (50% at 14 weeks), followed by focal clear-cell pattern (50% at 6 weeks), and no clear cells (50% at 3 weeks).
    • No significant correlation was found between survival and cytologic differentiation, histologic architecture, pleomorphism, or other specific cellular features.

    Conclusions:

    • The presence of clear cells in HCC tumors is the only histologic feature with significant prognostic value.
    • Clear cell patterns in HCC are associated with improved survival compared to tumors lacking these features.
