  • To identify independent predictors of poor sleep quality in patients with chronic obstructive pulmonary disease (COPD).

  • Secondary analysis of 106 COPD patients from two previous trials.
  • Overnight polysomnography, Epworth Sleepiness Scores, spirometry, and arterial oxygen tension (PaO2) were assessed.
  • Univariate and multivariate analyses identified predictors of sleep quality.

Main Results:

  • COPD patients exhibited poorer sleep quality and reduced rapid eye movement (REM) sleep compared to normative data.
  • Daytime PaO2 independently correlated with reduced sleep efficiency (P=0.041).
  • Forced expiratory volume in 1s (FEV1) correlated with arousal index, and age with REM sleep duration.

Conclusions:

  • Sleep quality is significantly impaired in severe COPD patients.
  • Daytime hypoxemia is an independent factor associated with decreased sleep efficiency in COPD.
  • Age and FEV1 also influence specific sleep parameters in COPD patients.
