Summary

Switching biologic treatments can effectively manage moderate-severe plaque psoriasis when initial therapies fail. Ustekinumab provided sustained relief for a patient with psoriasis and hand involvement, improving quality of life.

Background:

  • Biologic agents have revolutionized psoriasis treatment, but treatment resistance and loss of efficacy pose challenges.
  • Finding optimal therapies for moderate-to-severe plaque psoriasis, especially with hand involvement, remains difficult.

Observation:

  • A patient with long-standing, moderate-severe plaque psoriasis previously unresponsive to UVB, classical systemic therapies, alefacept, and adalimumab.
  • Adalimumab initially effective, but patient experienced loss of efficacy and severe hand psoriasis, impacting work and causing distress.

Findings:

  • Switching to ustekinumab resulted in rapid and significant improvement of psoriasis and resolution of psoriatic arthritis after two injections.
  • Sustained efficacy of ustekinumab was observed, with minimal disease activity and high patient satisfaction.

Implications:

  • Changing to a biologic with a different mechanism of action, such as ustekinumab, can be a successful strategy for patients with refractory psoriasis.
  • Effective psoriasis management with ustekinumab can significantly improve patient quality of life and ability to perform job responsibilities.
