Purpose of the Study:

  • To determine the reliability of ultrasound elastography (USE) in assessing solid thyroid nodules.
  • To identify factors associated with the reliability of USE.
  • To evaluate interobserver and intraobserver agreement for USE.

Main Methods:

  • Conventional ultrasound and USE were performed on 106 solid thyroid nodules in 78 patients.
  • Reliability was assessed using predefined criteria, including color distribution, score consistency, and signal loss.
  • Multivariable logistic regression analyzed factors affecting reliability; interobserver and intraobserver agreement were calculated using kappa statistics.

Main Results:

  • The consensual reliability of USE was 68% for the evaluated thyroid nodules.
  • Rim calcification, compressive force (≥3), and arterial pulsation were significantly associated with USE reliability.
  • Substantial interobserver (κ=0.738) and intraobserver (κ=0.765) agreement were found for reliable USE results.

Conclusions:

  • Ultrasound elastography demonstrates moderate reliability in assessing solid thyroid nodules.
  • Clinical application of USE should be limited to nodules with reliable USE results, considering identified influencing factors.
  • Further standardization may improve the consistency and applicability of USE in thyroid nodule evaluation.
