Higher cefazolin concentrations with intraosseous regional prophylaxis in TKA

Intraosseous cefazolin administration achieved significantly higher tissue concentrations than systemic dosing for total knee arthroplasty (TKA) patients. This regional antibiotic delivery method may improve infection prevention, especially against resistant bacteria.

- Intraosseous administration resulted in mean cefazolin concentrations of 186 ug/g in subcutaneous fat and 130 ug/g in bone.
- Systemic administration yielded mean cefazolin concentrations of 11 ug/g in both subcutaneous fat and bone.
- These concentration differences were consistent throughout the surgical procedure.
Conclusions:
- Regional intraosseous administration of cefazolin achieves antibiotic tissue concentrations substantially higher (an order of magnitude) than systemic administration.
- Further research is needed to confirm if this enhanced delivery translates to improved efficacy in preventing surgical site infections, particularly those caused by coagulase-negative staphylococci.
