Purpose of the Study:

  • To compare patient-reported asthma control with Asthma Control Questionnaire 6-item version (ACQ-6) scores in severe asthma.
  • To investigate the association between asthma control and emergency room visits.

Main Methods:

  • Cross-sectional study of 423 severe asthma patients in Brazil.
  • Patients completed the ACQ-6 and a subjective control assessment.
  • Data collected between August 2008 and March 2010.

Main Results:

  • Only 8% of patients perceived their asthma as uncontrolled.
  • 38.8% of patients had an ACQ-6 score indicating poor control.
  • Poor concordance between subjective perception and ACQ-6 scores was observed (kappa statistic).
  • Uncontrolled asthma was directly associated with increased emergency room visits (p < 0.001).

Conclusions:

  • Patient self-assessment of asthma control is unreliable and often overestimated.
  • The ACQ-6 is a more objective measure of asthma control.
  • Discrepancies in perceived control may lead to undertreatment of severe asthma.
