Summary

This study shows that cementless total hip arthroplasty with a titanium femoral stem has excellent long-term survivorship. The implant demonstrated reliable fixation and reduced complications in young patients, with bearing wear being the primary limitation.

Background:

  • Cementless total hip arthroplasty (THA) is a common procedure for hip joint restoration.
  • Evaluating long-term outcomes of novel implant designs is crucial for patient care.
  • Understanding implant performance, including survivorship and complication rates, informs surgical practice.

Purpose of the Study:

  • To prospectively evaluate the 5-13-year results of cementless THA using a specific titanium femoral stem.
  • To assess implant survivorship, osteolysis, intraoperative fractures, thigh pain, and cortical hypertrophy.
  • To compare the outcomes of this novel stem with existing second-generation uncemented femoral components.

Main Methods:

  • A consecutive series of 157 cementless THAs (142 hips evaluated) were studied.
  • Patients had a minimum 5-year follow-up, with a mean of 8.5 years.
  • Clinical outcomes (Merle d’Aubigné score, complications, thigh pain) and radiographic analyses were performed.

Main Results:

  • The 10-year survival rate was 99% for femoral and acetabular components, and 69% for the bearing.
  • Significant improvement in Merle d’Aubigné scores was observed postoperatively.
  • Low incidence of thigh pain (2%), no distal femoral osteolysis, and minimal cortical hypertrophy (3%) were noted.

Conclusions:

  • Cementless, machined, fluted titanium femoral components provide reliable fixation in young THA patients.
  • This implant design shows reduced thigh pain and intraoperative femur fractures compared to other uncemented stems.
  • Bearing wear necessitating exchange remains the main limitation of this THA construct.
