Summary

Routine colonoscopy inspection during insertion does not improve adenoma detection rates. Dedicated inspection time during withdrawal is as effective as splitting inspection time between insertion and withdrawal for finding polyps.

Purpose of the Study:

  • To evaluate the impact of additional inspection time during colonoscope insertion on adenoma detection rates.
  • To compare adenoma detection rates between strategies involving inspection during insertion versus withdrawal only.

Main Methods:

  • A randomized controlled trial was conducted in a university hospital and ambulatory surgery center.
  • Patients undergoing screening or surveillance colonoscopy were randomized to two inspection protocols.
  • Protocols involved either 3 minutes of inspection during insertion plus 6 minutes during withdrawal, or 9 minutes solely during withdrawal.

Main Results:

  • No significant difference in adenoma detection rates was observed between the two inspection groups (52% vs. 58%).
  • Total procedure time, polyp removal time, sedation doses, and post-procedure pain were comparable between groups.

Conclusions:

  • Inspection during colonoscope insertion does not provide additional benefit for adenoma detection compared to withdrawal inspection.
  • Current findings do not support routine inspection during colonoscope insertion in screening or surveillance colonoscopies.
