Rituximab effectively treats idiopathic membranous nephropathy (IMN), a kidney disease. This B-cell depletion therapy led to remission in most patients, improving kidney function and showing no serious side effects.

- A consecutive series of 100 IMN patients with persistent nephrotic syndrome were treated with rituximab.
- Remission was defined as proteinuria <0.3 g/24h (complete) or <3 g/24h (partial) with >50% reduction from baseline.
- Median follow-up was 29 months; renal function (GFR) and proteinuria were monitored.
Main Results:
- 65 patients achieved complete or partial remission, with a median time to remission of 7.1 months.
- All patients with at least 4 years of follow-up achieved remission.
- Significant improvements in GFR, serum albumin, and albumin fractional clearance were observed in responders.
- Proteinuria at baseline and follow-up duration predicted proteinuria decline; proteinuria reduction correlated with slower GFR decline.
Conclusions:
- Rituximab is an effective treatment for achieving remission in high-risk IMN patients.
- Rituximab stabilizes or improves renal function in IMN patients.
- The treatment demonstrated a favorable safety profile with no serious adverse events.
